New Client Form
FINRA requires M&W Financial to obtain basic client information within 30 days of the initial investment. In addition, we are required to maintain all information and obtain updated New Client forms every three years, even if no changes have occurred. Download, complete and mail the New Client Form if you are:
  • New as an investor partner to M&W Financial, Inc.
  • Aware of changes to your personal or professional information
  • Required to update the form as per regulations set by FINRA

Privacy Policy
Federal law requires M&W Financial to inform all clients how we collect, use, share, and protect your personal information. This policy is mailed to all investor partners each year.

Change of Address Form
FINRA requires M&W Financial to confirm, verify and follow-up with customers requesting a change of address.  Download, complete and mail the Change of Address Form to begin this process.  Please note we cannot accept a change to a P.O. Box without a street address to accompany it.

GUARDIAN OIL & GAS FORMS

Purchaser Questionnaires
Purchaser Questionnaires provide Guardian Oil & Gas and M&W Financial with valuable information for determining investor accreditation and suitability. Guardian Oil & Gas may not accept an investor partner that does not meet certain accreditation and suitability standards. For more information on accreditation and suitability, request a copy of our Guide to Direct Investing in Oil & Gas

Guardian Oil & Gas has two types of purchaser questionnaires, one for natural persons and one for entities:

  • Potential investor partners that are corporations, LLCs, partnerships, trusts, employee plans, IRAs, estates, or 501(c)(3) organizations should complete the Entity Questionnaire. 
  • Potential investor partners who are individuals should complete the Natural Person Questionnaire. 
  • Entities may qualify through certification of all equity owners, grantors, participants, or beneficiaries.  To accomplish this, the entity should complete an Entity Questionnaire, and each owner, grantor, participant, or beneficiary of the entity should complete a Natural Person Questionnaire.

Transfer Documents
When circumstances change, such as in death, divorce, or dissolution of an entity, it may become necessary for an investor partner to transfer his or her partnership interest to a third party. Guardian Oil & Gas, Inc. as Managing Partner, has sole discretion to approve or deny transfers of partnership interests. These transfer documents provide Guardian with the information it needs to evaluate a proposed transfer and make a prudent approval decision. Transfers to non-partners, or to entities not owned by partners, are rarely approved.

Sale Documents
In limited circumstances an investor partner may wish to sell or otherwise dispose of his or her partnership interest. This sale document provides a template to help facilitate a sale and/or disposal of a partnership interest. Please note, however, that all sales are subject to Guardian Oil & Gas’s approval, and securities laws place limits on resale of these types of securities.  Generally, sales to non-partners will not be approved.
